Instructions for PSFC Authors: Report Dissemination Form: "The Blue Form"

 

Once a report number has been assigned by the Library, a PSFC Report Dissemination Form (known as the "blue form") must be completed for each report, to ensure accurate tracking for the Library archives and for submission to ILP &  DOE.
